Entering Claim Data and Printing Your First Claim

NO NEED TO USE A “SAVE” OR “MEMORIZE” FUNCTION WHEN ENTERING DATA.  ALL DATA IS AUTOMATICALLY SAVED/MEMORIZED WHEN ENTERED INTO APPROPRIATE FIELDS.

Enter all data in CAPITAL letters.

Note: If you have entered a 4-digit year and it switches back to 2 digits, your Windows Regional settings have the M/D/YY display setting. To change the setting open the Windows Control Panel>Regional Settings>Click the Date tab>Change the Short Date Style field to M/D/YYYY.

Patient Insured Info tab – Step 1

When EZClaim first starts, it is showing the Patient/Insured Info input screen and is ready for new patient data.  This screen represents the top half of the CMS-1500 form, so enter data just as you would on a CMS-1500 form.

1.  Begin by clicking on the New Patient (F2) button, this will clear the screen, (if needed) and place the cursor in the patient's last name.  (EZClaim highlights the box in which you are entering data.) 

2.  The cursor is in the Last name box, type in the patient's last name.  When you are finished, press the Tab key on the keyboard.  This will move the cursor to the next field, which is the patient's First name. 

3.  Enter the first name and press the Tab key again.  Continue entering data and pressing the tab key until all the text data is entered. 

4.  Use the mouse to click on any boxes that may need a check mark by moving the mouse pointer over the little circle and click the left mouse button.  If you check a box and then need to remove the check, click the same circle again.  This will toggle the check on and off.

Physician/Diagnostic Info tab – Step 2

This screen represents the bottom half of the CMS-1500 form along with additional data entry features.

Continue entering data and pressing the Tab key or use the mouse to click on a box you need to fill in. 

Please Note: The following three items explain the areas on this screen that do not follow the CMS-1500 form exactly:

 

1.  Claim Form Default Values

Find the ‘Claim Form Default Values’ section located in the middle of the Physician/Diagnostic Information screen.

Enter into the Default section only those codes that remain the same for every date of service.  Example; Diagnosis, Place of Service, Type, etc.

Then when you are entering charge data on the ‘New Charges’ screen, click on the dates of the calendar and the default values will automatically be entered.

 

2.   Print Options

Setting these ‘Print Options’ will only affect the current claim form.

To set printing options as your Default Settings, see “Default Print Options” under Tools>Options.

      Summarize Claim Line Items - If the carrier requires line items to be summarized, place a check mark in this box.  For more information, see Printing Options.

      Date Format - Some carriers require different date formats.  You can select from 4 different formats.

      Currency Format - Some carriers require different currency formats.  You can select from two different formats.

      Advanced Print Options - Medicare has made some changes in regards to showing charges and line item date formats.  The Advanced Print Options button will open a dialog box that gives an additional 3 options for printing.

3.   Carrier Area

The carrier area box prints information at the very top right of the form.  When typing in this field, pressing the Enter key will move you down a line.  Press the Tab key to move to the next field. To enter Payers from the’ Payer Library’ click on the Carrier Area button. See Payer Library for additional information on setting up your Payer Library.

New Charges tab – Step 3

Click on the New Charges tab to enter dates of service.

Click on the dates on the calendar. (If you entered codes into the Claim Form Default line on the Physician/Diagnostic Info screen, those codes will be automatically entered.)

To enter codes manually put your cursor in the box and enter code. Tab across the line to enter data.

You may enter as many dates of service as needed.  If more than 6 dates of service are used, EZClaim will print multiple forms.

When you are finished entering date of service line items, make sure the CMS-1500 forms are in your printer and click the’ Print CMS-1500’ button to print the claim.

 

To print multiple claims go to ‘File’ on the Menu bar>Select Claims to Print

 

Date Formats

When you print the claims, the dates are formatted according to the Print Settings for each patient.  Go to Tools>Options>Default Print Options or see Default Print Options for additional information.
