Special Reports

EZClaim includes reports that require additional instructions.  Listed below are the special reports and instructions for each one.

Patient Statements

How do I generate a patient statement?

Patient statements allow you to print a statement that is suitable for patient billing.  Each CMS-1500 claim that is not marked as paid will print on the statement.  If the patient has 2 claims with 5 services each, then 1 statement will be printed with 10 services.  There are different statement formats but they all work the same.

IMPORTANT: A claim can be printed as a statement only if it is NOT marked as paid.

To print a patient statement:

1.  Click on the File menu and select ‘Reports’.

2.  From the report list, select the ‘Statement’ report.

3.  From the patient list, select the patient you want to bill.

4.  Click the Print or Preview button.

5.  Each claim that has not been marked as paid will print as a statement.

Accounts Receivable (Aging) Report

The A/R report will show the aging status on all claims that have not been marked as paid.  The report groups claims by patient and shows a patient aging as well as a grand total with aging data.

All aging is calculated using the Bill Date.  If the Bill Date is not available, it will use the following dates in order; Printed Date, Export Date, Current Date.

IMPORTANT: All claims not marked as paid will show on the report even if the balance due is $0.00.

Patient and Claim Notes

Can I track and print patient notes?

If you would like to keep patient notes, there are two places to enter notes.  Each Patient has a note field, which will hold 65,000 characters.  Click on Patient Notes’ on the Patient/Insured info screen. Each Claim has a claim note field, which will hold up to 65,000 characters.  To enter Claim Notes, click the Notes button on the Charges screen.  Each claim can have individual notes.

There are two reports included with EZClaim to print notes.  They are Patient Notes and Patient and Claim Notes.  The Patient Notes report will print only the patient notes and the Patient and Claim Notes report will print both.

Custom Reports

Since patient and claim data is stored in an Access 97 database, it is possible to write custom reports with a third party report writer.  We suggest using either Access or Crystal Reports to design custom reports.  Please note that we do not provide support on custom report designs.

Report File Location

The report files (*.rpt) must reside in the same directory as the EZData.mdb file.

Report Format

EZClaim uses Crystal Reports for the report format.  If you develop a new report using Crystal Reports with the correct format and filename, the report will be listed in the EZClaim program.

Criteria Determination

Each report name is followed by 5 characters of 0’s or 1’s (i.e. Claim List – 11100.rpt).  Each of the five characters represent the criteria used in the report.  The following chart lists each character position and the criteria used.

If using the All Data query in the reports.  Enter the words ‘All Data’ into the keyword summery field.

For example, the Claim List – 11100.rpt has 11100 for the criteria determination.  This report uses the date range to filter by service date, the facility criteria, and the patient criteria.
